**FOR PHYSICIANS**
As part of its mission
to improve the experiences and outcomes of Chiari patients, Conquer Chiari makes
available medically reviewed literature for qualified physicians to distribute
to both new and existing patients. This is a FREE service.
If you diagnose and/or
treat Chiari patients and would like literature sent to you , simply fill out
and fax the
Patient
Literature Request Form, or send an email to
director@conquerchiari.org with
your name, address, specialty, and how many Chiari patients you see per year.
Note that we send a maximum of 25 copies at a time.
